Atrial Septum defect (ASD)


Atrial Septum defect is a type of congenital heart disease in pregnancy (common type of birth defect in pregnancy). Atrial Septum is a wall in the heart which separates the right and left atria. Normally Atrial Septum defect is “hole in the atrial septum” in the newborn baby. Only surgical treatment can cure these problems completely.

Congenital Heart Disease In Pregnancy And The Role Of Fetal Echo

Congenital heart defect is the most common type of birth defect with which a baby has been born. It can affect the septum, heart valves, arteries and veins of the baby in question and affect the blood flow.

The causes for the CHD in baby can include medical conditions in mom such as diabetes, lupus, rubella, being obese and phenylketonuria. It can also be due to the changes in the genes or chromosomes. Fetus with certain genetic and chromosomal conditions such as Down syndrome, turner syndrome, noonan syndrome, velocardiofacial syndrome and alagille syndrome are also most likely to have CHD. Lifestyle habit such as smoking, drinking and talking certain medications during pregnancy can also causes CHD

Diagnosis
Severe CHD can be diagnosed during pregnancy or soon after birth; other can only be detected when the child gets older. During pregnancy a test called fetal echo is done by an experienced specialist to check the baby’s heart. It takes an image of baby in the wom, this test can be done as early as 18 to 22 weeks of pregnancy. The test is recommended for the following causes:


Maternal conditions
  • Diabetes
  • Anticonvulsant intake
  • Prev child with CHD
  • Infections : parvovirus, Rubella, Coxsackie
  • Autoimmune disease: Anti Rho/La positive

Echocardiogram

Echocardiogram is a test that used to create the image of the heart by using sound waves. This is used to manage and follow up the patient who have the heart diseases. It also used to evaluate the direction of the blood flow through heart. By using the electrodes which is placed on the heart, the electrical activity is measured.

The different types of echo cardiograms are : 

 

1.Trans thoracic echo cardiogram : In this this process, places probe, on the chest wall and bounces sound waves off the structures of the heart. The signals are received by the probe and converted to image with the help of a computer.

2.Transesophageal echocardiogram : Here instead of placing the probe on the chest wall, a cardiologist will direct the probe through the mouth into the esophagus. The sound waves can travel to the heart without the interference of the ribs and muscles of the chest wall.

3.Doppler echocardiogram: Using Doppler effect or technology, the echocardiogram can assess the speed and direction of blood flow, that can help to increase the amount and quality of information available from the test. The computer can add color to help the doctor appreciate that information.

4.Stress echocardiogram: The patient may be asked to exercise. The echocardiogram is performed before exercise as a baseline and then immediately afterwards.
